Nevada's Care Connection is a comprehensive information and service delivery system that aims to assist Nevada's aging and disabled populations access and navigate services available to them, and plan for their long term care.  The physical Care Connection sites (which are also called Aging and Disability Resource Centers or ADRC's) feature on-site specialists who can provide information and assistance regarding public benefit programs and other services.  As required by the Administration on Aging (AoA) and the State of Nevada, Aging and Disability Services Division (ADSD), these sites will provide, at a minimum: information and referral, options counseling and benefits access.


Who can Care Connection help?

Nevada’s elders, all persons with disabilities, their families and caregivers, and anyone planning for future long term support needs.

Why is Care Connection Important?

Care Connection helps consumers become aware of their choices, empowers them to make informed decisions, and facilitates easy access to public and private long-term supports and services.  The goal is to help consumers plan for future needs, but also to more easily access the right services at the right time, regardless of financial status.

What Services Are Available?

Nevada's Care Connection services include information and referral as well as benefits access and assistance, but centers on Options Counseling.

Options Counselors help consumers identify current and future needs.  In addition to the assessment process, they will assist a consumer in developing a plan for services.  Nevada's Care Connection can assist consumers to identify and access services for a variety of needs, including, but not limited to, the following:

  • Caregiver Support
  • Advocacy and Assistance
  • Housing
  • Personal Care Services
  • Transportation
  • Independent Living Services
  • Health and Medical Needs
  • Futures Planning
  • Employment
  • Recreation
  • And many more!
